It&#39;s basically a one-page form for either physical or mental disabilities (different forms). It&#39;s got all the Vet&#39;s info on it, the results of an examination, and a yes or no box is this Vet disabled and how much. Leave it to the doctors and not the desk jockies for the love of God.
