Try your unit administrator at your NG unit first. Your UA should be keeping these up to date and the soldier is suppose to get one each year.

I served in Active Duty first before going into the NG. I was supposed to get mine after leaving and had to call them in order for them to request it. Now it's locked up with my DD214.
You should contact the unit first. Chain of command. They'll help you or show you where to go next.

Does anyone have an answer to this? I have applied for a VA loan and being told I need a copy by the VA due to a change in policy. I did not need this form in the past to get a loan. I am in the middle of the loan process with a lender and hitting this roadblock.
