SPC Private RallyPoint Member 3558541 <div class="images-v2-count-0"></div>The literature online is very vague about where I should send the form. It says that I should send it to where I was trained which I assume is Ft. Sam, but I&#39;m not sure who to address it to and I don&#39;t want it to get lost. Where do I need to send my Federal Agency Verification for my state EMT license? 2018-04-19T12:38:59-04:00 SPC Private RallyPoint Member 3558541 <div class="images-v2-count-0"></div>The literature online is very vague about where I should send the form. I got mine through a county fire academy. Probably in your packet they sent you home with with all your certs and training creds. There was a gentleman I delt with at fort sam via email that was able to sign my verification and reciprocity form that I received by my county/state. His name was Jose Lopez. The AMEDC&amp;S requires the NREMT-A for the 68w.
