If you're looking to build a personal one, Amazon.com has a wide selection. If you're looking to replace Army issue kit, Ebay has the IFAC Pouch and refill kits. I loaned out my bandoleer once, as well as left a black fleece laying on a cafeteria table. Each one has been replaced by Ebay.

Talk to your unit supply Sergeant. They should know it's missing just in case. That's an important item, and you never know; they might have a spare lying around. Also talk to someone who has deployed. They will also likely have spares.

If you need to refil your army issued IFAK then all you have to do is go to your aid station and talk to your medics. If its your personal then what everyone has said in this thread are your best options

You can try to have one ordered through the supply system. If you are self building one, most of the components you should be able to get at your BAS. Here is a link for contents and NSNs of an IFAK. http://www.usamma.army.mil/assets/docs/IFAK.pdf Excluding the pouch, which is Class II, all the contents are Class VIII.

It's issued at CIF or RFI with your body armor or rifleman kit.
If you lost the one you were issued either acquire one off post at a surplus store. There's more than one version so ensure you get the correct one. Your other choice is to fill out your statement of charges and get it reissued.

As a Medic and EMT, I would tell you to never buy a pre-built IFAK. These small pouches can be bought in any size and you can put whatever you want in them. Why be forced into custom size and pay for mostly mediocre items that many don't work well? Sure it might cost more but for an IFAK, making your own custom-built will be best.
