I am not sure if they would be able to reprint you one out. If that system is still even in use. I think the form was a DA 268 if I am not mistaken. Maybe add the group pool, or Motor SGT, or maintenance ... they would be the ones to ask. They are maintained by the Battalion Master Driver. The Army no longer uses the ULLS system anymore. It is all online now. But the transition hasn't gone well if you didn't get a copy. You should have been given a DA 348 that has your qualifications on it. More than not they are not maintained very well. The Army is trying to do a better job at this. The issue that you are going to run into is that I don't believe it tracks historic data. Once you are purged from your unit's roster that should also drop you from their drivers roll and in turn knock out your record. If your battalion Master Driver has your 348 you can request that but more than not they don't really track those very well and it could be shredded by now. I would say that you are going to have a struggle to get this if you are even able too. I know this because I was one. Big issue is that most of the master drivers I knew didn't do so well at keeping records. And I believe they only maintain those records for only about 2 to 5 years. If they say they don't know that's probably the truth. Your best bet is to go yourself to the battalion master driver yourself to check to see if they maintained your info. But I'm sorry to say it may all be a waste of your time. If you and your unit meet that criteria, they can upload and restore a backup from whenever you were there, print your DA Form 348E and then simply restore a backup from that current date and it would be as if nothing happened.
