What kind of supporting documents do I need for the Drivers Badge?

I currently have found out about the drivers badge, and I do qualify. If my research is correct, I have to fill out a DA 4187 and submit a DA 384-E (Drivers Record) and 5984-E (Operators Card) . What else do I submit? Is there a form or something that I can put down my miles? Does anyone have examples on what they sent?
Thanks in advance, no one from my unit knows about it and Id like to know the process for myself and to assist other soldiers.

Any commander at the LTC/O5 or higher can award the deiver’s badge. Once the Soldier meets the requirements per AR 600-8-200, par 8-20; the orders issuing authorities just publish whatever (orders, 4187, etc...). Just need the unit’s master driver to endorse and submit thru appropriate channel the packet.

Before GCSS-A you could pull a report out of SAMS-E and ULLS_G before that by driver and get miles, it was contingent that you put the right miles in on the 5988E when you,closed out the dispatch.

I don't know the GCSS-A functionality but your Master Driver should. 2016-ish there was a push to put all Master Drivers in GCSS-A so they could do this management.

You have your Operator's Permit (not card), DA 5984-E, and that is signed off by your current commander.

Whoever created your current document should be able to use the dispatch records to determine how many miles you have driven and what vehicles were used. If the documentation is not complete, i.e. the dispatch miles haven't been calculated and added to your SAMS-E record, that's a problem for all the drivers.

Check with the issuer of your Operator's Permit and see what they can dig up out of the computer. If your unit is still using paper records and/or has borrowed vehicles from other units, you may have a project on your hands. Yes, I was a SAMS-E operator for a while.

Once you have complete documentation, you should apply to your PAC section for the award and if you qualify, you should get the award(s).

Your Unit Master Driver has record of you. Once you meet the requirements IAW AR 600-8-200, a memorandum is created and needs to be approved by a LTC or higher. Once it is signed, you get a copy of the memo and S-1 should input it into eMILPO.

I just received this award. Per the regulation that others have provided to you, you only need a memo with your name, rank, social security number and the memo should specify its for the Drivers Badge. I believe it was sent up to my BC and he signed off on it. After that I took it to my S1.

You need either validation of miles driven or have had your military driver's licenses for a year with no issues, from my understanding of the regulation. So don't let them try to mess you over saying you need back date records of every single mile driven.

Driver badges is awarded through chain if command Only. If I was you I ask my squad leader then plt.sgt.

Many folks have already mentioned Master Driver. You say no one in your unit knows how to do this. But I bet Master Driver does - or can figure it out.
