What is the best course of action to get your records moved to new city and name changed on files?

I need to update my VA account and records to reflect the change in last name and moved from my previous city for VA Medical purposes to my new city/state. The VA site states that this has to be done in person, but I haven’t been able to get in touch with anyone at any of the local RAPIDS sites to see when they will be opened to do this.

Look through the VA site for name change forms. And I think to change from one VA hospital to another you just have to schedule an appointment to get a new PCP assigned. With all that’s going on that might be a problem.

Call the VA hospital in your new city and ask to he transferred to enrollment. Tell them all your info including corrected last name and ask to be assigned a primary care physician. They will ask where you were seen before.
