He should have the template for it and you can find the course details in the ATRRS Catalogue by searching Under the MOS tab for 68W30. Fill it out so that reflects the course and dates you want and turn it in through your training room for your Commanders signature. What caught me off guard when I attended in 2012 was the Common Core requirement (a distance learning course). I don't know if this is still applicable, but it was required for graduation credit (even after completing phase 1 and 2). We had to get help getting slotted for that course from the school house directly.
