My Chapter 2 appointment results in my DD 2808 showed my distant vision was 20/25. The reviewing officer checked block 74a "Qualified for service - Retention" "Is not qualified for service - Commission", in block 76 he indicated "Failed distant vision" ICD 2-13b. Block 78 he refers to AR 40-501 Chapter 2. This is where the issue is; The Chapter 2 paragraph 13b is from an outdated Reg revision., the most current AR 40-501 does not have a paragraph 13b. I would have tried to put in a DA 4187 ETP but on what Reg do I use ? I have called LHI and they told me to have my POC to call in, my POC said he called in, I am now like a cat chasing its tail. What do you advice I do?

If it was filled out incorrectly, take it back to the provider, show them the reg and anve them fix the error

The examining physician completed the form correctly but the reviewing Officer is a contracted LHI provider (I believe). It seems the chain of events is as so: A facility completes the form and leaves out some specific blocks to be completed by a reviewing officer, and sends the form to LHI. LHI then sends the form to the Reviewing Officer, once it has been reviewed it is send to LHI to send to the RHRP. There is a disconnect between the Primary facility physician and the Reviewing Officer, this is because I initially called the primary facility that conducted my exam and the had no idea who that reviewing officer was.

SGT (Join to see) - MORE INFOR: I therefore can not get the contact of that Reviewing Officer to fix this error because he is not employed by the facility I visited for my exam
