The Intermediate Care Technician was a step in the right direction; however, it only takes advantage of the experience and skill of the traditional, basic military “medic” or brings the civilian EMT up to that level. It fails to tap the experience, education, and experience of the IDMT (USAF) or IDC (USN) that was designed to operate in remote and austere environments, as the only medical provider for miles or days. Tapping into this resource could open up countless numbers of account or urgent care appointments, in some cases still using a physician preceptor model, but freeing up physicians for more complex cases. Due to the needless restrictions in civilian practice, there is a wealth of untapped knowledge and experience as healthcare providers who are working as customer service reps at LOWES rather than become an underutilized component of the wasteful and inappropriately managed US EMS system. With the ability to perform advanced assessments, perform and interpret diagnostics, place referrals, limited prescription authority, and perform procedures (among a number of other things), why not improve access and cut cost by tapping into this otherwise wasted resource? But a doctor in Florida va can’t see the actual swallow study or MRI I had done at Albuquerque va . <br />Referrals to a clinic should be for a period of one year . Several years ago a orthopedic surgeon wrote I need both knees replaced and to call when ready . Tge doctor is still at the va . I am being made to go back to primary care for a consult . Your doubling and tripling the work on primary care doctors . I have a known condition . I should just be able to call ortho clinic and make the appointment. I have much more . . <br />My spine is destroyed . I have been to 12 surgeons accross the country . VA surgeons already said it’s too complex for them . Three different surgeons in three states . Send veterans to the BEST surgeons not the closest . A neurosurgeon may do a lot of brain surgery but not spine surgery . This leads to bad surgery which cripples veterans even more . Send me to cedars Sinai in California. No 1 sound team in the nation . Just down the road from then is west LA V/A . Transfer me there for recovery and rehab . That saves money . You can. There is always a local va hospital to recover and rehab in to save money . Response by 1LT Larry Bass made Oct 30 at 2024 4:37 PM 2024-10-30T16:37:49-04:00 2024-10-30T16:37:49-04:00 COL Randall C. 8897489 <div class="images-v2-count-0"></div>My question is regarding VA/DoD Resource Sharing Agreements (RSA)* and their limitations and/or execution/understanding by VA physicians and the administrative staff.<br /><br />Specific to my situation - why can&#39;t I take a radiology or laboratory order from a VA Community-Based Outpatient Clinic (CBOC), which is co-located with a Military Treatment Facility (MTF), to the MTF&#39;s radiology/laboratory department to be fulfilled (especially if there is an existing sharing agreement between the CBOC and the MTF)? Instead, VA facilities must be used regardless if there is a delay or travel involved to use them.<br /><br />Background:<br /><br />In order to expand its capacity to provide outpatient primary care, the VHA has established approximately 1,400 CBOCs under the regional VAMC. Many of these CBOCs are co-located with a MTF and have an established RSA.<br /><br />I don&#39;t know what the RSA covers for other facilities, but specific to my situation (I am assigned to the Washington VA Medical Center and see my primary care at the Fort Belvoir VA Clinic*) there might be a disconnect in execution.<br /><br />The specific use case/example that I&#39;ve experienced is I would see my VA Primary Care Manager (PCM) for a complaint. The PCM might order blood work and/or an X-Ray. My CBOC schedules all lab draws as the facilities are limited and those appointments are usually a few weeks in the future unless there was a cancelation. In order to have any radiology performed you must travel to the VA Medical Center (VAMC) the CBOC belongs to which for me is about an hour average travel.<br /><br />In most cases I throw up my hands in frustration and go outside the VA system and/or use an urgent care facility.<br /><br />The oddest part is that the MTF will do labs and radiology from outside physicians as long as I provide them with the order (I have TRICARE Select for my family, and they have had labs and x-rays performed numerous times by bringing in the provider&#39;s order). Every time I&#39;ve requested an order from the VA provider or their staff, I&#39;ve been informed that it&#39;s not possible and I must use the VA&#39;s resources.<br /><br />I researched if there was a RSA in place and was able to see on the VA/DoD Medical Sharing Office (MSO) webpage* that one between the VA clinic and the Belvoir MTF had been established. Were told we have to be rated at100% for many of these appts like dental and podiatry. Yet when we&#39;ve appealed our disability ratings 4 or more times over 30 years and finally get the 100% rating, we still can&#39;t get these appts. What happened to the policy that if we couldn&#39;t be seen withing 30 days by a VA provider that we&#39;d get a community care referral? It&#39;s not happening. When I specifically asked for a referral due to the lengthy appt. delay, I was refused. Wholistic health? We can&#39;t get dental care, yet we have a new dental clinic in Knoxville Tennessee. I&#39;m told the VA has a hiring freeze. Why pay a lease and utilities on a building when it can&#39;t be staffed?<br />The VA is wasting humongous amounts on paper. For instance, 1 prescription comes with at least 6 pages of paper with our personal data printed on each page 5 times. Likewise, when we go to a community care appt, the facility receives mounds of paper with our personal information, a lot of which isn&#39;t even pertinent to that particular appt. Oftentimes, staff at the facility will discard most of the paperwork. So where does fraud, waste, and abuse come in? When Physical Therapy ordered me a special scrub device for my feet for the shower, I received scrub brushes that are used in the Operating Room, but they cant be returned. My particular brush had to be reordered, so more expense and delays. Last year I was scheduled to get an exam. This was arranged by one of your contractors. I had to cancel the exam because my wife needed cancer surgery. 2 months later I was abLE to attend the exam. I contacted the original contractor telling them so. My file had been returned. It was then given to another contractor, fine. The first contractor had me going to a doctor in Denmark. The second contractor said they had no doctors in Denmark thus sending me to Germany. Worst part was the second contractor has me talking to a person who&#39;s english was so bad I could not understand this person on the phone. Why can&#39;t these contractors work together? You have do idea what I went through to get the exam. When it is all said and done, you no longer pay travel expenses for us Vets under the FMP, but if I was in the States, yes I get travel, WHY? Are we just second rate Vets because we live OCONUS? Let&#39;s say the letter is dated 1 October, yet I receive the letter on 20 October (which has been the norm) I am behind the eight ball to begin with. The very first thing I need to do is request an extension which takes time. Can&#39;t the VA figure out or give a different time for those of us on FMP, say 45 days? Would be more realistic. Maybe someone from the VA needs to live OCONUS for a while and understand what we have to go through. Also I’m court ordered to pay my ex-wife $2,200.00 alimony in cash a month. I also have to pay the SBP out of my active duty retirement, which is $182 a month out of my military retirement and was ordered to keep it in her name as beneficiary.<br /><br />I retired at 61 years old because my health is going down hill quickly due to all my service connected disabilities. I received 100% disability, due to the Pact Act and six deployments in the Middle East. I petitioned the family court to remove my $2,200.00 mandatory court ordered alimony. They refused and deducted only $200.00 because I am military and my wife deserves 1/2 of my 100% Veterans Affairs Disability Benefit. I was the individual that deployed, not my ex wife. I have talked with people within the DAV organization, they told me it was illegal for my ex-wife to receive any disability benefits, unless I owed child support or in the rears on alimony. Neither of them exceptions are in play, my children are all over 30 years old. My ex-wife received her alimony pay every month on the first day of the . The DAV told me they couldn’t help me! I also contacted the VA and I was redirected to a service representative, she agreed with the DAV’s answer. My medical records and history were shared contrary to my consent or direction to non-healthcare individuals with no right or need to know causing grievance harm. <br /><br />Nurse practitioners routinely made absurd diagnoses that I had to get corrected with MD’s. I have stopped contacting the IG as it seems clear there is no accountability and it is a waste of my time to request help. This is the ‘tip of the iceberg’. <br /><br />Each VAHCS seems to be its own relative fiefdom and the command climate varies from HCS to HCS. In my experience, only half of these VAHCS have patient advocates that will even respond to a patient. In one incident I was told contrary to Federal regulations and state law that HIPAA prevents them as mandated reporters from reporting financial exploitation or abuse of vulnerable individuals. The law is clear that their inaction constituted a misdemeanor criminal infraction. VA regulations clearly states that their position is completely wrong, and yet they stubbornly committed the crimes of omission. Nothing was done by the VA to address these crimes. <br /><br />The APA ethical standards used to dictate that only Psychiatrists and Psychologists make mental health diagnoses. Now numerous clinicians (social workers, nurse practitioners and the like.) with a fraction of the qualifications or vetting are allowed to do so. The PhD’s at NAMRL in Pensacola were livid when the DoN willfully violated this in the 1990’s. The basis for their outrage has not changed but the permission structure has. This seems to be a function of monetary and budget considerations rather than what is best for the veterans and service members. It harkens back to the disgraceful incident of disabled dazed veterans with traumatic brain injuries found wondering around unsupervised on the grounds of Walter Reed. Corrective action was taken then. <br /><br />My plea is that it be done again and that a blue ribbon panel investigate the shortfalls in care that exist by having under qualified caregivers in all aspects of VA care. I further ask that those unfit to be in health care be held accountable and purged. There are countless examples of averice driven decisions harming patients in the private sector. Please ensure that budget decisions are weighed carefully. If the budget is insufficient, then lobby congress and enlist the help of us veterans if necessary.<br /><br />There are personnel that Mother Teresa would have admired. Get rid of those at the other end of the spectrum. I finally reached out to VA homelessness program in El Paso, TX, and received lackluster assistance from VA program staff, Ashley Lamell, the SSVF affiliated agencies, both Endeavors &amp; American GI Forum. Case management was deplorable, rapid housing was &amp; still is nonexistent, &amp; custodians with job titles like Case Manager, Housing or Employment Coordinator, and Director/CEO of Agency, failed to deliver program objectives. Sent several emails &amp; made calls to inquire about progress in housing, employment, etc., pertaining to my case. Was met with remedial responses, victim blaming and shaming, and at times, told &quot;No, our agency isn&#39;t allowed to cover moving expenses.&quot; And things of that nature. But what incensed me more, mu case Manager emailed, 7 days prior to March 1st, when rent was due, that I had exhausted nine months of monies/rental financial assistance &amp; dropped from their files w/out proper notification, given list of names &amp; tel phone numbers to reach out for assistance. This lead to an immediate EVICTION NOTICE, which was filed &amp; complied with by Judge, and causing remarkable dent in my credit file let alone my rental history. Waa this homelessness program meant as a joke? Be ause guess what? Guess who is homeless again? I am. Not one director, CEO, case Manager bothered to do their job as outlined in CFR. At which point the VA determines if it&#39;s a valid claim or not. If the veteran requests a supervisor review of the claim and they find that the VA made an error in determining the denial of benefits then that re evaluation of the review is then processed under the same and put back into the que as if it were a new claim. This creates a double jeopardy for the vet as they have probably already waited for the initial review and now they have to wait for the new review again when it was identified as an error on the part of the VA.<br /><br />Request that when an error on the part of the VA has been identified that those claims no longer be placed in the standard que for review but escalated to the top as the Vet has already waited for the standard que but now, due to no fault of the veteran, they have to wait again.<br /><br />Background: using Community Services I had Left Knee Replacement done. I asked about the process for increasing my rating as we get a temporary 100% until the vet heals. I was told to refile my claim, which I did but was given the wrong info as I should have filled out a different Temporary request vs a claim after the fact. My initial claim was denied, I then I submitted a review. They found that the VA had made an error, 15 months after my initial denial I was awarded the Temporary 100%, their excuse was the Surgeons Report wasn&#39;t submitted, yet I have fax confirmation logs from the Surgeons Office showing they were submitted, on top of that there was an Explanation of Benefits showing the Surgeon had been paid for services, yet my claim was denied. Granted in hindsight I was provide incorrect information, if I had had the surgery in the VA Hospital, my benefit would have been almost Immediate, yet as a Vet I had to wait 16 months after my surgery to receive the temporary 100%.<br /><br />As the VA Secretary what can you do to improve Vet engagement for Temporary 100% for those members in Community Care? What can you do to improve records management from Community Care Providers to the VA System? Obviously if a Surgeon gets paid but no Surgery Report is in the veterans records something is wrong! What is the check and balance? Finally, what can you do to improve veteran outcomes when the VA identifies an error for a veteran due to the VAs oversight, lack of oversight, records management, failures in VA systems, or failures in the Community Care Records, or other VA issue that caused the initial denial. <br /><br />As stated, veterans who submitted claims have already waited their turn in line to be denied, yet upon review a member of the VA found that a mistake was made, so putting the veteran back at the end of the line for the same claim they already waited on just doesn&#39;t feel like the VA is placing the veteran first when they made a mistake!<br /><br />How can you fix this so vets don&#39;t face double wait times for claims they have already submitted but errors were found during the review? This has happened to me twice, both times due to Community Care Records Management issues.<br /><br />Thank you. Anyway I’m trying to get my PCP to help with DBQ’s per VHA Directive 1134, ever time I file a claim for PTSD, lower back pain, and diabetes. I get rejected, I just recently found out about Nexus letters and DBQ. I was under the impression that the VA looked at my VA health records. Congress wrestles with how little they want to increase our pay and then take some of it back in taxes. Instead of taxing us when they want to give a 3% pay raise make it 2.5% and not tax us. I also believe they need to get rid of the Survivors Benefit Plan. It is really set up bad and should be a reduced life insurance policy that can go to the members dependents and not just his wife. The GSA mileage reimbursement rate for all government employees, including va employees, is approx 50% more than the rate veterans are reimbursed for their out of pocket mileage reimbursement rate. The government transportation office estimates that the average cost per mile to operate a vehicle is in fact at the gsa rate and IRS deduction rate. How is it veteran centric to take money out of the pockets by NOT paying the fair reimbursement rate? I am SMC (T) and I meet the qualifications for the program due to Neurological Residuals for my Multiple TBI&#39;s. I have turned in all kinds of documentation for the program up to and including my decision by a VA law judge that states I need my wife as my caregiver or I would be in a memory care nursing home. It has taken them until this last rejection to actually let me know that I don&#39;t have 6 months of prior medical records and that is why they are denying me. Even though I had my doctor write a note literally stating that my wife has been my care giver the entire time I have been in his care which is over 2 years. They failed to do their jobs when I first got here and didn&#39;t give me a TBI worksheet which would have allowed me to start building my documentation about my TBI and getting access to the right doctors to treat me in the Multi Trauma Unit. My question is this why is this program so difficult and lacking in transparency that we have to go through years of denials when we meet the qualifications of the program. In 2020 I was awarded 100 T/P. The claim is now basically about 28 years of back pay. There has been so much that has happened, but the Reader&#39;s Digest version is this... My attorney believes the claim will be back paid to 1992. (I&#39;m hopeful but not holding my breath, because this part of the claim is headed back through the regional Boise VA from the BVA. The Boise Regional VA has been a nightmare.) <br /><br />First they have managed to break up my original claim and turn it into about four separate claims. In the process, I&#39;ve been called a liar (and my claim denied) by Boise VA personnel several times regarding my claim, because until 2011 the fact that the USS Tarawa was almost hit by a scud missile in Desert Shield/Storm was classified. They kept telling me it was impossible because the Tarawa was never in theater, and I either don&#39;t remember correctly or was lying... I was called a liar by Boise VA Personnel because the Boise VA couldn&#39;t locate my Walla Walla VA medical records pertaining to me participating in the Gulf War Control Group when I was discharged. I was even awarded a zero percent disability by the Walla Walla VA in regards to the study back in 1992, but the Boise VA took it away from me when I moved, because they couldn&#39;t &quot;find&quot; my Walla Walla records. They said, &quot;There was no Gulf War Control Group&quot; and the disability was issued by mistake. Eventually the Walla Walla records were &quot;found&quot; all-be-it several years later... Nothing changed, other than they don&#39;t say I&#39;m mis-remembering or lying anymore. <br /><br />Hell I was even awarded 50% for a portion of the claim. It was granted under the legacy claim going back to 1992, but the Boise VA refused the back pay because they said it was a new claim. There was no break in the claim and it even shows &quot;GRANTED&quot; under my legacy claim on VA.gov. The VA answer? File another appeal and start the process all over... <br /><br />Last month I had a hearing with a BVA Judge, it was the first time in this claims process I felt I have been treated fairly. The BVA Judge was awesome. That said, apparently the BVA Judge can only grant service connection, now unfortunately (if service connection is granted) the case goes back to the Boise Regional VA to determine effective date. Because of my choice to go there, I am sent to the VA hospitals that CBOC is affiliated with (West Haven and Newington VAMC&#39;s in CT). Those VAMC&#39;s are 1.5 and 2.0 hours respectively, away from my location. So, there are some VAMC services that I need without being sent by my PCP, so I use the Providence, RI VAMC for that (ie: Psych, Dental, ER visits...). What has happened to me MANY times is on several different occasions, one VAMC needs to know about a visit at another VAMC. A full understanding of my medical history will not be known unless one VAMC goes in an old outdated system called Joint Legacy Viewer (JVL). <br /><br />Solution: In this day and age, there is simply no reason why a Veterans information (complete record) shouldn&#39;t be IMMEDIATELY available without having to jump through hoops or outdated systems...regardless of what VAMC he/she goes to! Indeed, since Viet Nam, we have been an all volunteer force. I analyzed data for leadership my entire 22 year career. I have studied the VA Annual Suicide Reports and the VA efforts to reduce US veteran suicides starting in 2020. Your analysts could more clearly tell you, and America&#39;s leaders, the real situation regarding the increase in veteran suicides during the last twenty years. Since 2001, according to VA data in the VA Annual Veteran Suicide 2023 Report, the ratio of veteran suicides compared to veterans alive has the gone up (about 6000 suicides in 2001 with 22 million veterans alive; compared to about 6300+ suicides in 2021 with 18 million veterans alive). My analysis shows an increase of about 30 percent during those 20 years. I believe the dollars the VA has spent to reduce veteran suicides during those years was unsuccessful. To be objective, i will commend the VA for these changes: Veteran Crisis Line, Vet Center(s) creation, increased mental health programs, increased funding to mental health programs in general.<br /><br />However, in my opinion, the VA has not listened to veterans about how to PROACTIVELY help veterans in crisis. In my own case, two suicide attempts since 2020, I have observed your offices not wanting to hear about this failure. Nor do your offices have any notion of more effective ways to help us.<br />My contact with numerous VA employees show that your organization listens to Florida State University (FSU). (FSU is not even ranked in the top ten psychiatric colleges in the US.) I was challenged by a VA employee of a Suicide Prevention Team to study works Dr Thomas Joiner of FSU. In reading a book of his about suicide, published in 2024, it&#39;s my opinion he does not understand what drives veterans to suicide nor how to proactively help prevent our suicides.<br /><br />With the above comments in mind, my questions to VA leadership and you:<br />1. Why isn&#39;t a leading psychiatric college, like DUKE, giving the VA advice?<br />2. Why isn&#39;t the VA actively listening to veterans and possibly achieving better success, rather than the dismal failure of the last 20 years?<br />3. When will VA analysts actually tell VA leadership in clear, understandable reports with graphs which portray the consistent upward (bad) trends of US veteran suicides?<br />4. However, it seems despite the number of officials who&#39;ve cycled through leading the VA, we continue to face many of the same problems and difficulties with processes, people, and systems...year after year...cumulatively impacting the degree and standard of care provided/received. Many health organizations outside of the VA, while &quot;off the chart&quot; expensive, provide world-class care and support. We&#39;ve been in desperate need of VA leaders who have the authority (and the budget) to be visible change agents. Generally, we aren&#39;t facing &quot;new&quot; problems in receiving support. Your own data, metrics, procedural tracking, and KPI&#39;s should support that and drive more data backed change and decision-making. Again, you have volumes of feedback to rely upon. <br />These Town Hall&#39;s, while perhaps informative on some level, shouldn&#39;t be opportunities to gain new insight. Consider providing the veteran community a prioritized list of the core items where you&#39;ve made progress, current state, and what the future holds...key action items you&#39;d classify as &quot;must wins&quot;. We need the VA to help us...and clearly, we need to help and trust each other. Many of us have battled on foreign soil. Many of us have battled wars at home...both psychological and physical. But we shouldn&#39;t have to battle on our own soil for the help we now desperately need as a result. Get people inside the system who know how to get things done...especially veterans. I know these aren&#39;t easy problems to solve and there&#39;s no single panacea, but these individuals (veterans) know how to work difficult issues and aren&#39;t afraid to challenge. <br /><br />All of this must start at the top with you/leadership being unapologetic about the change you want to see, and doggedly determined to see those changes through with a results orientation. On 23 Sept 2024 I received an electronic payment in my bank account. After 30 days of calling numerous VA phone numbers I spoke with a representative that said it was a payment for my overpayment. I asked to get documentation for my personal records. The representative said they can’t do that. I called back 2 other times and last time I spoke with a “supervisor” and was told they couldn’t print/mail me anything. I even asked if they could email me and was told no. It would be nice if I could log into my VA payment information and see all my payment history. For example, if your highest disability is 30% for a fractured hip, and another disability is rated at 20%, your VA rating will be 40%, not 50%. I was rated at 90% total for a couple of disparate medical problems, but the VA calculates that to be 60%. It&#39;s like saying that - Sure, the first bullet did the most damage, but then you got shot somewhere else the next day, and the second one will only be half as significant. The VA insists that putting all ratings into a black box disability calculator to determine the overall rating, rather than just adding all percentages is somehow fair. What is the rationale behind that? I attended the Art Institute in Colorado for almost 4 years using the majority of my 9-11 G.I. Bill benefit. The school closed without any advanced notice prior to me (and many others) completing an degree and graduating. I applied many times for restoration of my G.I. Bill benefit; however, each time it was denied. I am currently on the docket for the highest appeal that I believe I can submit. The department of education has declared that the Art Institute misled and defrauded students over many years. The department of education has already forgave all federal student loans for any student who attended the Art Institute during the same timeframe as I attended due to this. If I would have taken a federal student loan instead of using my 911 G.I. bill I would owe absolutely nothing. I do not understand why my request for restoration of benefit was repeatedly denied. The passing of H.R. 1767 will restore my entire 9-11 G.I. I wrote a point paper about the Home Hemodialysis even explaining that this method may have a high start up cost but in the long run saves the VA money. VA pays for my dialysis but now they don&#39;t have to pay facility or staff cost. Why does my wife have to give up her employment to provide me life sustaining, lengthening, and enhancing specialized care while saving the VA money and not get selected as a home care provider.
