If she has had that, and is up to date on her immunizations, her PCM should be able to sign the paperwork for you. Enroll them into EFMP and they’ll have a questionnaire to fill out and an appointment with their PCM to ensure that all of the care that is required for them is available at the installation. The packet will get signed there and then you’ll have an appointment with the EFMP Dr. who will fill out, sign, and stamp your 5888. From there you will turn that into your S1 to turn into the command sponsorship office. Once command sponsored, they will be added to your orders so that travel is provided. Once you get there you’ll fill out a 4187 to change your COLA rate to include your dependents. It’s been a few years since I’ve gone through the process but that’s should be pretty accurate. Anyone feel free to correct if I mixed up any of the steps. <br /><br />Is she with you on base or back home? Even though Alaska is part of the 50 states, it is considered an overseas tour. If you don’t get the screening, all medical and dental will be out of pocket. You won’t receive any on post privileges for her to include the daycare, housing, and COLA. Having the EFMP Screening is beneficial to her. When you schedule her appointment, make sure you state “EFMP Screening for Alaska.” Good luck to you...
