How do I initiate a medical discharge from the Reserves?

I left active duty 31DEC2018. I had signed on for 3 years of reserve duty that started 01JAN2019. VA rated me 50% 20FEB2019 and re-rated me 100% 22FEB2020. Now ACFT is coming and my broken body is screaming at me. How do I initiate a medical discharge?

You cannot initiate a medical discharge. It is initiated by a medical provider putting a 3 on one or more of your PULHES (profile). The Army is not medically removing Soldiers at this time just because there are events that a Soldier cannot complete on the ACFT. You'll be finished with your 3 years before you have to take the ACFT for record. Why not wait it out and just ETS?

I guess I should have acknowledged the fact that my injuries have left me in a position to not provide a good role model for my soldiers. Some days I can barely move from my back injury. How am I to instruct the junior enlisted assigned to me, without being a hypocrite. How can I justify counseling them for failing when I am failing? The desire is still strong Within, ultimately the body has failed my desire

You should provide your provider at your unit a copy of the records that reflect your rating. Your provider may have be able to recommend you for a MEB. The issue most of the time in the Reserves is that they don't have a copy of your medical records that show you are not able to deploy and maintain your readiness.

You can only turn in your paperwork up on the health issues- Medical folks decided what to do about an MEB
