How do I go about getting a filled out USAREC 3.1 post physical?

First off, thank you for taking the time to read this. Over the past year I have been getting together my packet for 153A and finally sent in my application a few days ago. I recently received a response stating my packets deficiencies and I am confident I can correct all but one of those deficiencies. On the recruiting website it states that if applying for 153A I need to only send in the 2808 with the stamp for the approved physical however now they are telling me I need the UF 3.1 cover sheet as well. Any way to get just the UF 3.1 filled out from where I took the physical or do I need to schedule myself for another flight physical? I appreciate any response, thank you!

Take the UF 3.1 with you to your flight physical they’ll fill it out for you and once you’re complete your physical info will be sent into the AERO system which will automatically be sent to fort Rucker to get verified by the flight physician. That’s also when you’ll get your stamp.
At least that’s my understanding of it

Okay I did not read your question correctly so disregard. Also just take a UF 3.1 back where you got your physical with the 2808 and have them fill it out.
