How can I have an HKK discharge code (drug use) and a 488 SPN code (personality disorder) when I never failed a UA or psychiatric evaluation?

I'm wondering why/how I could get HKK 4 888 when I passed the piss test and the psychologist was cause the CO was pissed at me and wanted to prove I did drugs - but psychologist after interview me, closed his case file and said ok off the record have you every done any drug or even know anything about - - I said no - - I guess I answered the question about getting the munchies was wrong. I never saw his file but I have requested all my personal and med records - - they said 10-2016
