I have submitted for a compassionate reassignment back in October 2016 with all the required forms and letters from providers and chaplain. I keep asking for a status on where it is without a definite answer. I spoke with the medical director with EFMP at my location and she contacted One of the EFMP coordinators at HRC and was told it still has not been submitted.. what can I do now ?

you need to walk the dog on the chain of handling and see where it was, where it is now.
You handed it to?? go see that person, what did they do with it? then go see that person / shop.. continue till you find it, or understand it has disappeared .
If you find it, politely, professional ask if there is a hold up and if so what can you do to fix it?

if you get to a dead end, no file found and no recored of file transfer to another shop, place, admin location.. you have two choices, throw a fit and whine..or accept its gone, go get the copies you made (you made copies of everything yes??) recreate the packet, resubmit, and then stay one step ahead of it.. you know the likely path now... the next person is who you want to monitor, letting them know, "hey morning, just checking to see if the xyz got my packet to you yet?" if not there in few business days.. go to the last place and ask

This is an expedited request and is required to be actioned, if you've been waiting since October then there is an obstacle in the way - someone sure as hell is not doing their job! At this point, the time for talk is over, if the Company/BN cannot get you an answer, ask to see the BDE CSM on the open door policy. CSM Wafe is a professional and a Soldiers Soldier, he WILL get you an answer!
