For those on active duty, what are the fields on your dog tags?

I will be using this for an article. My old dog tag had name, SSN, Blood Type, and Religion. What is on yours, and what is the date it was issued? Are you on active duty and will pass an inspection with those dog tags?

These days it's Name, DODID Number (instead of the SSN), Blood Type, Religious Preference. I got this set last year I think, when we switched from SSN to DODID.

I was issued a pair in Basic during Reception with my Name, SSN, Blood Type, and Religion.
I got another pair with the SSN replaced with my DODID Number.
I'm National Guard but same issued tags as my AD brothers in IET.

Mine were issued with: name, blood type, ssn (but now it's dod#), and religious preference.

I've had mine since 2008. They have Name, SSN, Blood Type, and Religious Preference. I've seen some people get the most ridiculous stuff as religious preference such as Jedi.

My era was 1994-2016: last, first MI; line 2 Social; line 3: blood type; line 4 religion. On the red medical tags lists a 5th line with the medical alert/allergy. These tags made it through every SRP and command spot inspection I ever went to.
