Does anyone have an example of a memo for SLC? I will need to send a 4187 to HRC with an endorsement by the first General officer.

News to me. I was kicked out for getting injured during the APFT in SLC. My situation was that I had to wait 6 months before I was enrolled again. I was then automatically enrolled and no letter from anyone was needed. And of course I went back and passed with flying colors. This was only last year too.
