Does anyone have a template for the Commander's memo allowing for a flight physical that they can share with me?

I am getting my flight physical together for a flight packet. Last time I did a flight physical was on Fort Campbell so those memo's were easy to find, however I am on a base with no aviation units and don't remember what to put on the memo.
