Does anyone have a copy of a 4187 to return to ALC after being dropped?

Long story short i was dropped from ALC back in 2018 and I ended up getting promoted to SSG due to the covid ETP does anyone know how long I have to return back to ALC before losing my rank and does anyone have an example as to what it should look like? I have tried S3 and tried email HRC with no luck. Thank you in advance!

I put the phrase "covid ETP promotion" into Google and found the answer in 10 seconds
