Does anyone has a memo or what form a soldier would need to request a second job while still on active duty?

I see that I forgot to put 'job' I have corrected that mistake. I am looking for information for another soldier. I am trying to see if their are any forms required or just a memo for the commander.

I am assuming you mean "a second job". There is no specific form for the request, however, seeing as how it is a Commander's decision, I would recommend preparing a memorandum for him/her explaining what the job is, why you need it, how it will/will not affect your work performance, etc. Make it clear, concise and to the point. When you meet with the 1SG and CDR to discuss it, be prepared to answer any/all questions, and come prepared with the necessary documentation needed to answer those questions (LES, LORs, Job Proposal, etc.). The more organized, prepared and professional you are, the better it will look for you.

If "second job" isn't a correct assumption, you can ignore what I just said.. lol

Since this is posted in jobs you can assume he means a second job. In that case different units may have different policies regarding off duty employment. Your best action is to inform your support channel of your intent and request that they provide you feedback on how to proceed as the rules will vary based on unit policy letters.
