Does anybody have a digital copy of CA NG Form 137-1 Unit clearance form?

it would appear that California as it's own unit clearance form - I am working to submit my retirement packet which asks for this form - but nobody (including the unit) is able to provide me a blank copy to complete. I have ask about using the NGB form or the DA Form and the answer I get is -"that's not the right form" - really. Any help in this matter will be greatly appreciated.

finally got the unit to send me the form - the DA form - so that is the one I submitted - we will see what they say.
