It is all about the needs of the NG and for AD the Defense Health Agency (DHA). Few years back when DHA first took over they were cutting some 1800 healthcare positions so to direct commission then would have been nearly unheard of. The guard has relatively few RNs and the vast majority outside of AD are in USAR.
