It's a counseling.
Your signature confirms that the content of the counseling form was the content of the discussion in that counseling, and that you were present for it.
Are you changing your mind about one of those two things?

Was it a monthly counseling or an event oriented one? Your objections for an event-oriented you need to state and record under Session Closing. Otherwise, if you agreed to the plan of action or felt that something is not stated that would change anything in the counseling statement; then it is too little too late (unless you were coerced).

Look your signing means just that the Name, rank, etc is correct, and that you received said counseling- NOTwhether you you liked it or agreed. Don't agree then rebutt it in writing, proving that it was wrong- not that it hurt your feelings, etc.

Just as you don't want to hear your SGT berating you about past problems you've solved,
he doesn't want to hear continuous whining about something you agreed & signed already.
