The operative word is Can be. Not will be. A lot of circumstances involved when it comes to a chapter procedure. I had a soldier who failed two pt tests. Being a medic I noticed something during the test that others didn't. In a nutshell, I referred him to a physical therapist who prescribed orthotic inserts for his shoes. after that no problem for the run.

Yes, you can be discharged from the Army for failing to 2 record PT tests. No, there is nothing you can do about it for your Soldier. The APFT is an individual event and if they are unwilling to improve or maintain outside of PT hours, then that is their own fault. They will of course, not receive a dishonorable discharge, it's been a few years since I kicked someone out for APFT failure, but, I believe it is a General discharge.

PFC Jeffrey Herrington I tell my soldiers, there is nobody in the Army that’s going to care about your career more than yourself. If you can not maintain the standard of 60 in each event, then you are negatively affecting the units readiness.
PRT is not designed to increase PT score but is intended to maintain the current state of physical fitness.
It is always encouraged to conduct PT outside of work to improve
