For the UCMJ if the command approves you are fine. For the Host nation it depends if their medical governing body recognizes their credentials and they want them preforming surgery. Army regulations only apply to people in the Army. Here's my best guess as to an answer:
1. A non-US doctor, licensed in his home country can perform medical procedures in his country. Same doctor may not be able to perform medical procedures in a US military medical facility in the doctor's home country.
2. A US licensed military doctor can normally can perform medical procedures in US military medical treatment facilities wherever they are located. The same doctor may not perform medical treatments in non-US military medical facilities outside the US.
3. Under some conditions US military doctors may be invited to perform medical treatments in foreign hospitals due to emergency conditions or humanitarian considerations.
