Who determines how much con leave is given to a solider if the surgery is being done off post?

So do I need to schedule an appointment to set that up or will they already be tracking SGM?

SPC (Join to see) - I assume the MTF referred the surgery off-post.
Your MTF is responsible for generating your DA 31 based of the recommendation and the DA 3349.
You need to talk to your commander and the provider who referred you for off-post surgery.
And check out AR 600-8-10, ch 5-4 (page 25) before you get started.
