I left active duty after 4 years, but subsequently started drilling and then spent a year Active Duty for Training. My DD-214 dates from my original active duty discharge. My question is - do I need to request an updated DD-214 to reflect the additional active duty time (which, again, was ADT)?

You would not be updating your 214 but you could get a new one. DD214 is a summary of duty over a specific time. Depending on they type of training and the length, you can qualify for a new DD214. Check your HRC portal and ensure that the dates of duty are reflected - that is the record for reservist in addition to a yearly point summary. (It is different for the National Guard, they don't give DD214 at all.)
