I made a lot of goof ups when I was in the Army. In basic we went on our 20 mile force march and bivouacked that night. The next morning when me and my shelter half partner woke up it was bright sunlight and everyone was gone. We thought oh shit, we're in trouble now. We caught a ride with a mess truck coming to clean up the area our company had marched to. We ended up being the ones to clean up the area and were put on KP the rest of the day.

I would say the goof up I made in my career, was when I was a new private and was told by my first line supervisor to go to the supply room to find a prick E-6.

I was at at my first AT and was tasked with replacing all the batteries on an M88 tracked recovery vehicle when I knocked a wrench off the access and down in between the hull and the engine. Naturally I was required to find the wrench and spent about two days doing it. Learned a couple very important lessons here 1 tie 550 cord to your tools when working on important/complex machines 2 watertight works both ways and 3 don't be that guy.
