Does anyone have an example or know where I can find an example counseling for jump refusal?

One of the Soldiers in my company became a Jump Refusal and I was tasked out by 1SG to counsel him. But I have no clue as to what this counseling should include in it or how it should be typed. Help please!

Just state the facts
That he was on active jump status as of XX XXX XXXX (talk with the BN S1), and had requested / volunteered for a jump assignment.
DTG and location he was notified of the jump.
DTG and location he was put through/ present for/ accomplished pre jump and name the JM that led the training
DTG and location he assembled for the jump
DTG and location he was issued parachute
DTG and location he boarded the airplane.
DTG and by who he was given an order to exit the airplane.
If a second JM gave a following order , that person name as well...Or if the same JM, then state that he was given an order to exit a second time.
That he was re moved from the stick, given an order not to touch his gear.
DTG, location and who inspected his gear upon landing
And that nothing was found faulty with his equipment.
The point of all that is each of those DTG's prior loading the AC identified a recorded opportunity for him to terminate...
If the CDR is intended to run him though NJP, format your counseling statement and have your BDE legal clerk run it past JAG ..if it passes with no edits, complete the counseling session.
