Be it OER, NCOER, or whatever, have you ever received an evaluation that you thought was lower than what you deserve? What did you do to fix it? What was the end result?

Actually yes. I won...much. But it is not something you do lightly. And you should consider that God created Time in Grade for you to ponder things like this. So be sure you are right, and don't fall over your sword over just a difference of opinion. Accept that some people may see something you don't, or may see things differently than you do. Learn from it. However, there are always cases when people want to use an NCOER as a counseling tool or a hammer, and that is very wrong. I'm fond of reminding people that if you don't stand for something you will fall for anything! But you are still a soldier, your job is to follow orders, but not to the point of suicide.

I have never appealed an evaluation. I have included a rebuttal/comments with one.

An appeal is a pretty tall hurdle, at least on the O side. You have to be able to PROVE that the rater was WRONG. Since the evaluation is essentially subjective, that is a very hard thing to do.
