Does anyone have any knowledge, or can provide any guidance, about using SLRRT?

My battalion leadership has directed me to send out the SLRRT (Soldier Leader Risk Reduction Tool) to help monitor the level of risk in Soldiers' lives. I found the form and the guide on its use, but nowhere did I find exact guidelines on what constitutes each level of risk.
Does anyone have more information on this?

The primary purpose of the SLRRT is to inform and foster discussion in a one on one counseling session. Treat the information on the form as informative for your leader book when going into a counseling session, or use the form as a kind of guideline when checking in on a Soldier regarding their welfare.
What I think that it does best is serve as a kind of punchlist, reminding you to broach topics and ask questions you might not think about.
I can give some suggestions on it's application, if you'd like.

SGT Christopher Churilla - Depends on how well you know your Soldiers, but if you're going in cold, invite them to do a self-assessment and discuss with you during counseling how they view themselves. Compare that to what you know about the Soldier, and use that to inform how you will interact with them going forward on some of these subjects.
With a Soldier who has been identified as at-risk, use it to help exhaust and identify the potential stressors in their life and develop resources to buttress and strengthen their ability to cope with those stressors.

SGT Christopher Churilla Pulled this off the web. Sounds like a very weighty responsibility to put on a Chaplain's assistant. There is a lot involved and should be handled at each unit level. This reference also contains guidance on what constitutes low, medium or high risk factors.
